Good Luck Shortlisting to commence 11th December Main duties of the job The Care Coordinator within an acute care ward will be responsible for coordinating aspects of patient care including liaison with other agencies both internal and external to CWP. The post holder will be the central coordination point forward based multi-disciplinary activity, organising and supporting with ongoing regular meetings to facilitate progress to recovery and discharge with relevant CWP team members, relatives and carers and other external agencies. Knowledge of Office Administration/Custo mer service acquired through training or experience to NVQ level 3 or equivalent Knowledge and Expertise Knowledge of Health and Social Care administration and processes. Knowledge of Microsoft office applications, internet and email Knowledge/familiarity with medical terminology Experience Able to work well in a team. Experience of working in an Office Environment. Experience/knowledge of issues of working with confidential information Experience of working with healthcare professionals/previous experience in the NHS. Working in a busy and demanding environment.
