Locations: Yeovil (1 x 37 hours per week, 1 x 22.5 hours per week) Bridgwater (37 hours per week) Starting salary £37,280 rising to £41,771 per annum Hybrid working options 30 days annual leave What will I be doing? Work in the office a minimum of 2 days per week; the remainder of the week may be done using technology in a hybrid model. Utilise your skills and experience to support and empower residents of Somerset to remain independent for longer. Tailor solutions around residents’ needs, providing continuity of support and intervention. Promote independence in everything the team does. Work alongside Health and Social Care colleagues (as part of the neighbourhood PCNs). Liaise with key partners to facilitate improved provision of services, team training and recruitment. Involved in developing, shaping, and building integrated care local services. Requirements Be a qualified Social Worker and be registered with Social Work England.
