This approach allows people to return home as soon as they are medically stable, where their care needs can be assessed in a familiar environment. Responsibilities Providing short-term support to individuals recently discharged from hospital. Helping assess their ongoing care needs in the comfort of their own home. Supporting a process that reduces hospital stays and avoids unnecessary long-term placements. Collaborating with health and social care professionals to determine the most appropriate next steps, whether that’s reablement, continued care, or full independence. This is a rewarding opportunity to make a real difference in someone’s recovery journey, helping them regain confidence and independence in their own space.
