Care Assistant - Home to Assess Based in Rochford Salary: £13.20 per hour + mileage at £0.30 per mile Driving licence and own transport essential Guaranteed hours available (20-37.5 per week, subject to availability) Shift patterns: 7am-3pm / 3pm-11pm Must be available to work across 7 days, including alternate weekends About the Care Assistant role: Supporting Recovery at Home We’re proud to support individuals transitioning from hospital to home through our Discharge to Assess (D2A) model. This approach allows people to return home as soon as they are medically stable, where their care needs can be assessed in a familiar environment. Responsibilities Providing short-term support to individuals recently discharged from hospital. Helping assess their ongoing care needs in the comfort of their own home. Supporting a process that reduces hospital stays and avoids unnecessary long-term placements. Collaborating with health and social care professionals to determine the most appropriate next steps, whether that’s reablement, continued care, or full independence. This is a rewarding opportunity to make a real difference in someone’s recovery journey, helping them regain confidence and independence in their own space.
