Main duties of the job We are looking for a passionate, committed band 6 midwife to provide maternity care within our community midwifery team based at Darlington Memorial Hospital. You will be responsible for the planning, implementation and evaluation of individualised care plans which ensure delivery of holistic, research-based care, prioritising communication, informed consent and relationship building. CDDFT community midwives are passionate about facilitating antenatal and postnatal continuity of care and you will be supported as an autonomous practitioner to plan your diary around your caseload, ensuring that women and families are at the centre of everything we do. About us If you are being interviewed you must accept an interview slot in the system to continue, even if you have arranged with the manager. You must be able to produce ALL certificates stated essential in the person specification or you will not be able to complete pre-employment checks. We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes. We particularly welcome applications from disabled and Black, Asian and Minority Ethnic (BAME) candidates as BAME and disabled people are currently under-represented. Job responsibilities Our team at Darlington and Newton Aycliffe provide care to women and families across a varied urban and rural area. We have a very diverse case load, including vulnerable and isolated communities, providing opportunities to make a real difference to the families in your care. We have a well-established and well-led team of midwives within the team, who you will find very welcoming and supportive. The team are assisted by the wider maternity team, including a dedicated Community Matron and Matron for Public Health and Inequalities. We are very keen to encourage community midwives with an interest in providing care across the pregnancy and postnatal journey to apply to work with us to deliver outstanding community based Antenatal and Postnatal Care to the mothers and families in this area. We are also committed to provision of our home birth service and would particularly welcome community midwives with an interest in further developing this important area of care. We are looking for an individual with a passion for health promotion, as the role of community midwife at our Trust includes working within the UNICEF framework for Infant Feeding, and close working relationships with our specialist Antenatal and Newborn Screening Team to deliver all elements of the National Screening Programme.
