Provide evidence‑based care for patients with long‑term conditions, with a primary focus on diabetes. Manage chronic disease programmes (diabetes, asthma, COPD, CHD) and implement a diploma‑level diabetes pathway. Assist and perform routine tasks directed by nursing staff, GPs, and associated healthcare professionals. Conduct cytology, wound care and dressing management, spirometry, routine and childhood immunisations, contraception checks. Support health promotion, self‑care and motivational interviewing initiatives. Collaborate with the multidisciplinary team to meet care needs of housebound and residential home patients. Adhere to the NMC code of professional conduct, local and national policies, guidelines and protocols. Person Specification Essential skills: Ability to provide quality care; good interpersonal and communication skills. Experience: Working experience in general practice (required). Qualifications: GCSE grade A‑C in Maths and English; qualified in an NMC‑approved nursing degree. Desirable qualifications: Phlebotomy, wound care, immunisations, asthma and diabetes knowledge.
