Due to retirement, Measham Medical Unit has an exciting opportunity for a highly motivated, enthusiastic and committed Practice Nurse to join our friendly team. A special interest, or qualification, in Asthma / COPD management is desirable.The position is for 30 hours per week and to be worked Wednesday - Friday.We are looking for an experienced practice nurse who isconfident and comfortable in managing long-term conditions as well as deliveringtreatment room services. You will bewell supported by your nursing colleagues and the wider multi-disciplinaryteam.Salary will be dependent on the experience of the successful candidate.Main duties of the jobThe purpose of this post is the deliveryof high quality primary care nursing services to the Practice population. Youwill work with Practice policies, protocols and guidelines whilst usingexperience and clinical judgement to provide nurse interventions and care.Working within the NMC code the postholder will deliver care that is evidence based and cost effective.To be responsive to patients needs andcontribute to improving quality standards.Work collaboratively with themulti-disciplinary General Practice team to meet the needs of the patients.About usMeasham Medical Unit is a well-established friendly, supportiveand forward thinking GP practice based on the outskirts of North WestLeicestershire. We are dedicated to providingsafe, high quality and patient focussed care to our patient population of15,800 residing in North West Leicestershire, Staffordshire, Derbyshire andWarwickshire.We have a large clinical team comprising of 6 GP Partners,Salaried GPs, ANP, ACP, Physician Assistants, Clinical Pharmacists, PracticeNurses, HCAs and Nurse Associate.The clinical team are supported by management andadministration teams who have a wealth of knowledge and experience.We are a Dispensing Practice and dispense to around almosthalf of the practice population.We are committed to a collaborative and supportive workingenvironment and promote a healthy work / life balance for all members of ourteam.Job responsibilitiesPrimary key responsibilitiesThe following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:Clinical:a. To work independently and accepting professional accountability within the scope of your clinical practice and expertise.b. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practicec. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeingd. Sign post effectively to other clinicians and agencies when appropriatee. Promote health and well-being through the use of health promotion, health education and screeningf. Apply infection control measures in all areas of practiceg. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSFh. Undertake clinics and offer focused lifestyle advice in line with local, national guidance and Quality Outcomes Framework indicatorsi. Provide wound care (ulcer/Doppler, etc.) to patientsj. To review patient medications to enhance compliancek. Manage patients with Long term conditionsm. Assess primary and secondary cardiovascular riskn. Asthma/COPDr. Adult immunisation including seasonal/flu vaccinations. Cervical Cytology screening for patients eligible for the national screening programmeCommunication:a. Communicate effectively with patients/families/carers regarding information about their care and how results of tests should be obtained, considering differences in language, literacy and culture.b. Demonstrate excellent interpersonal, verbal and written communication skills.c. Work collaboratively with the multi-disciplinary general practice team and other external organisations.d. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)e. To support the clinical team with all safeguarding matters, in accordance with local and national policiesf. Maintain accurate and contemporaneous records using the SystmOne computer system.g. Be aware and comply with all data protection requirementsPersonal / Professionala. NMC registrations upholding professional standards of practice and behaviour.b. Maintain confidentiality with regard to information pertaining to patients and staff.c. Provide support and mentorship to colleagues in order to assist other to develop knowledge, skills and abilities.d. Participate in continued professional development/training to ensure knowledge and competencies in all aspects of the role are maintained.e. Complete all mandatory trainingf. Monitor effectiveness of own practice by participating in peer review/reflective practice and ensure NMC revalidation requirements are met.g. Promote a professional approach at all times by dress and attitude to patients and colleagues.h. Attend and contribute to various Practice meetings as required. (Some of these are mandatory and the only reason for not attending will be annual leave, study or sick leave).i. Facilitate a positive learning environment for students and staff, giving and receiving feedback in an open, honest and constructive way.Secondary key responsibilitiesIn addition to the primary key responsibilities, the Practice Nurse may be requested to:a. Participate in local initiatives to enhance service delivery and patient careb. Support and participate in shared learning within the practice in order to improve patient carec. Continually review clinical practices, responding to national policies and initiatives where appropriated. Participate in the review of significant and near-miss eventse.
