Responsibilities Provide treatment, advice and teaching within professional competence. Support patients, families/carers in their own homes, residential care homes and in nurse‑led clinics. Liaise with GPs, other professionals and statutory and voluntary agencies. Operate under the supervision of the District Nurse. Have a full UK driving licence, access to a car and be able to travel to a variety of sites, including weekends and some bank holidays. Essential Criteria and Qualifications Registered Nurse (Part 1) with NMC registration. Full UK driving licence and access to a car. Effective member of a multi‑disciplinary Primary Health Care Team. Knowledge of key issues in Primary Care Services. Excellent verbal and written communication skills. Demonstrated ability to innovate and motivate. Work within and across professional and organisational boundaries. Commitment to the provision of high‑quality evidence‑based care. Benefits This post attracts a High‑Cost Area Supplement of 15% of the basic salary (minimum £4,714 to a maximum £5,941 per annum, pro‑rata for part‑time). Probationary period applies.
