Hackney’s Early Help Service is seeking a compassionate, dynamic Early Help Family Support Practitioner to join their skilled and motivated team.

About the Role As an Early Help Family Support Practitioner, you will play a vital role in delivering high-quality, child-centred interventions that empower families with multiple and complex needs. Working within a wider multi-agency system, you’ll provide early, intensive support that promotes resilience, strengthens parenting capacity, and helps ensure children can thrive.

What You’ll Do Deliver high-quality, family-centred support
Work collaboratively within a team of Early Help Family Practitioners to deliver timely, impactful interventions.
Carry out holistic family assessments using evidence-based tools to understand adverse childhood experiences and factors affecting family functioning.
Develop child-centred plans with clear outcomes, targets, and review dates, using Hackney’s Early Help Assessment Form.Help families build resilience and achieve sustained outcomes
Provide bespoke support to children and families through one-to-one work, home visits, group sessions, and targeted interventions.
Promote and strengthen attachment between parents and children, enhancing overall child development and wellbeing.
Respond resourcefully to risks and early signs of poor outcomes, making full use of community resources and settings to reduce isolation and enhance support networks.Lead and coordinate multi-agency collaboration
Act as the lead professional for families, coordinating information sharing and support at Multi-Agency Team (MAT) meetings.
Facilitate and lead Team Around the Child (TAC) meetings, ensuring that plans are robust, coordinated, and responsive.
Work in accordance with Hackney Family Support Practice Guidance and the Multi-Agency Team protocol.Safeguard and advocate for children
Create a safe environment that protects and safeguards children, ensuring all safeguarding procedures are rigorously followed.
Identify and address barriers to positive parenting such as housing, financial issues, and access to services.Empower parents and carers
Coordinate and deliver parenting groups to strengthen parental understanding, confidence, and skills.
Support parents/carers through tailored interventions that enhance their capacity to meet their child’s needs.Work flexibly and collaboratively
Participate in children’s centre and wider service activities, contributing to an inclusive and supportive environment for families.
Adapt to the needs of the service, including occasional evening or weekend work when required.About You We’re looking for someone who is:
