Paramedic Practitioner – Amersham Vale Practice Job posted 11 November 2025. Salary: £55,000 – £60,000 per year. Contract: permanent. Working pattern: full‑time. Job Summary The Paramedic Practitioner (PP) will act autonomously within the professional scope of practice, providing care for patients face‑to‑face, by telephone consultation and home visits. The PP will operate in a multidisciplinary team, prioritise and triage patient needs, prescribe as appropriate, and ensure safe, evidence‑based care in line with NHS and NICE guidelines. Main Duties & Responsibilities Provide clinical care with general practitioners, advanced nurse practitioners, clinical pharmacist, practice nurses and healthcare assistants. Patient contact via patient‑facing clinics, triage, on‑call duties and home visits. Conduct home visits, post‑discharge reviews and manage minor illnesses. Prioritise and triage patient needs, making referrals for investigations and secondary care as required. Carry out clinical practice at an advanced level using expert clinical skills, critical judgement and knowledge. Deliver care in line with NHS, NICE guidelines and evidence‑based practice. Coordinate planning and delivery of care, ensuring patients and carers are involved. Prescribe and review medication within the scope of practice, following clinical protocols. Diagnose and manage acute and chronic conditions, including drug‑ and non‑drug‑based treatment. Request and interpret diagnostic investigations (e.g., X‑ray, blood tests, scans) within agreed protocols. Assess, diagnose and treat patients in practice premises or in the patient’s home, referring to appropriate agencies when needed. Work as an autonomous practitioner, upholding personal and professional clinical standards. Enhance performance through continuous professional development and share knowledge with colleagues. Participate in audit, evaluation and implementation of practice change to meet patient needs. Contribute positively to the effectiveness and efficiency of the team. Person Specification Qualifications – Essential Qualified paramedic or specialist paramedic (FDSc or BSc) and registered with HPCP or holding a post‑registration certificate Level 3 in first‑contact care. Experience in chronic disease management and mental health. Completion of Stages 1 and 2 of the Health Education England roadmap for practice map (https://www.hee.nhs.uk/our-work/allied-health-professions/enable-workforce/roadmaps-practice-0). Qualifications – Desirable Accredited training in chronic disease management. Non‑medical prescriber qualification for independent or supplementary prescribing. Mentoring/teaching qualification MSC level. Experience – Essential At least two years in primary care or a similar setting. Experience in triage and managing undifferentiated or undiagnosed conditions. Experience delivering minor illness and injury management. Evidence of service improvement and utilisation of NICE guidelines. Experience in audit and research; at least five years leading a healthcare service and knowledge of NHS principles and values. Ability to work independently and as a team player; recognise priorities and reference to senior clinicians. Experience – Desirable Evidence of service development. Familiarity with electronic clinical recording systems such as EMIS. Experience in teaching & training, mentoring and coaching for change. Telephone triage expertise. Chronic disease management experience. Experience collaborating across health and social care boundaries. Skills & Attributes – Essential Strong inspirational leadership and coordination skills.
