The postholder is an Advanced Clinical Practitioner, acting within their professionalboundaries. They will provide care forthe presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinicaldecision-making and expert care for patients within the general practice. Main duties of the job HealthEducation England describes advanced clinical practice as delivered byexperienced, registered health and care practitioners. It is a level ofpractice characterised by a high degree of autonomy and complex decisionmaking. This is underpinned by a masters level award or equivalent thatencompasses the four pillars of clinical practice, leadership and management,education and research, with demonstration of core capabilities and areaspecific clinical competence. Advanced clinical practice embodies the abilityto manage clinical care in partnership with individuals, families and carers.It includes the analysis and synthesis of complex problems across a range ofsettings, enabling innovative solutions to enhance peoples experience andimprove outcomes. This jobdescription covers many elements of the role but is not exhaustive. There is anopportunity in this role to develop skills and services within Beccles MedicalCentre (BMC), including managing key clinical areas, training, and research. About us Beccles Medical Centre is an established GP surgery, in the Suffolk market-town of Beccles. The Practice has a list size of approximately 20,000 patients. We have 5 Partners, and just over 100 additional staff. We are a GP Training practice and also host other students studying at the nearby University of East Anglia. We work very closely with our PCN. Our mission is to provide high quality healthcare with respect and compassion, ensuring that the patients are at the heart of everything we do Job responsibilities DUTIES &RESPONSIBILITIES The post holderis responsible for the following: To delivera high standard of patient care as an Advanced Clinical Practitioner in generalpractice, using advanced clinical skills, and a broad and in-depth theoreticalknowledge base. As a memberof GP consultants Team (with various clinical skill mix) to either care forthem or if this is not appropriate triage them to an appropriate appointmentwith another more appropriate clinician; To appropriatelyassess, examine, investigate, diagnose and treat patients with acute or chronicpresentations. Resulting in the safemanagement and/or appropriate referral, following practice guidelines andprotocols Order/ interpretinvestigations, laboratory tests, x-rays and act on results as appropriate The practitionerwill need to prioritise and triage the needs of patients accordingly making anynecessary referrals for investigations in the appropriate manner. Patients withproblems needing referral to secondary care could be discussed with the dutyGP, where appropriate, who is supervising or if not available at an appropriateinterval another regular GP in the practice before making such referral. To manage knownexacerbations of chronic illness ensuring appropriate assessments, treatmentand where appropriate onward referral to appropriate specialities facilitatingself-care and/or care within the community setting. The post holderwill work closely with the inter-professional team to support new ways ofworking that emphasises a more efficient and patient focused service acting asa professional clinical role model To play an activerole in the development/ improvement and audit of processes that facilitatesand improves patient flow. Practice withconfidence and competence within appropriate nationally agreed professionalguidelines such as NMC/ RCN / HEE Guidelines, the Health and Care ProfessionsCouncil (HCPC) and the College of Paramedics (COP) To ensure thataccurate documentation and records of patient care are kept. To providespecialist advice on patient care to the Practice team taking into account allaspects of clinical governance, and act as a specialist resource within thePractice To enhance theclinical services offered by the practice 2. JOBDIMENSIONS To manage ownclinical workload in general practice responding effectively to patient needand ensuring patient choice and ease of access to services. To work effectivelyand within the boundaries of professional practice as part of a clinical teamdelivering services to meet patients needs. Each ACP atBMC will have a minimum of THREEareas of main clinical practice from the following options (but they may choose more than 3 to create a portfolio role): Acute Home Visits Urgent Care / MinorAilments Clinics Routine Clinics Intermediate Careward duties (Beccles War Memorial Hospital) Long TermConditions Clinic / Visit Personal specialty(e.g. womens health, minor surgery etc.) Each ACP at BMC will have an agreed area of leadership responsibility from the followingoptions: A Clinicalspecialty aligned with QoF (e.g cardiovascular medicine) A Clinicalspecialty aligned with CQC (e.g. safeguarding) Management area(e.g. nurse lead) QualityImprovement support role (e.g. audit lead) ServiceDevelopment support role Each ACP at BMC will have an agreed area of education and or research responsibility from thefollowing options: Research Medical Studentteaching GP Registrarsupervision Care Navigatorclinical upskilling 3. PRIMARYDUTIES & AREAS OF RESPONSIBILITY A. CLINICAL PRACTICE: The post-holder will : Practise incompliance with their respective code of professional conduct and within theirscope of practice, being responsible and accountable for their decisions,actions and omissions at this level of practice. Provide the choiceof direct access to a prescribing Practitionerat home, in the Practice and remotely, for the general practicepopulation Provide a firstpoint of contact within the Practice or in the Community for patientspresenting with undifferentiated, undiagnosed problems, making use of skills inhistory taking, physical examination, problem-solving and clinicaldecision-making, to establish a diagnosis and management plan Instigate necessaryinvasive and non-invasive diagnostic tests or investigations and interpretfindings/reports The post holdermust be an independent prescriber: to prescribe safe, effective and appropriatemedication as defined by current legislative framework Provide safe,evidence-based, cost-effective, individualised patient care Offer a holisticservice to patients and their families, supporting people in making decisions,planning care or seeking to make positive changes, with an emphasis onprevention and self-care Promote health andwell-being through the use of health promotion, health education, screening andtherapeutic communication skills Refer patientsdirectly to other services/agencies as appropriate Work withmulti-disciplinary team within the Practice, and across the broader healthcarecommunity, to promote integrated and seamless pathways of care Contribute to thepractice achieving its quality targets to sustain the high standards of patientcare and service delivery Participate inidentification of community health needs and develop patient/family-centredstrategies to address them Help develop andset up new patient services and participate in initiatives to improve existingpatient services. B. EDUCATION, TEACHING ANDMENTORING The post-holder will : Maintain a clinicalportfolio that evidences advanced health assessment skills and competencies. Inconjunction with their clinical supervisor formulate a personal developmentplan to evidence a commitment to continual professional development. Using agreedcompetency framework appropriate to the individuals scope of practice. Participate inregular performance review with the Clinical Supervisor Demonstrate andmeet competencies to deliver appropriate health care such as history taking andphysical assessment. Participate incontinuing professional development opportunities to ensure that up-to-dateevidence-based knowledge and competence in all aspects of the role ismaintained To undertakefurther specialist training as dictated by service needs and the changingrequirements of service provision. Promote a learningenvironment for patients, nurses and other health professionals. Support the widerteam to build capacity and capability through work-based and interprofessionallearning, and the application of learning to practice. Contribute to theplanning and implementation of the teaching for existing staff, such as medicalstudents, Nurse students, GP registrars, Health Care Practitioners, within thePractice. Provide supervisionand mentoring to students and other members of the team C. PROFESSIONAL(incorporating Leadership and Management and Research) The post-holder will : Promoteevidence-based practice through the use of the latest research-based guidelinesand the development of practice-based research. Monitor theeffectiveness of their own clinical practice through the quality assurancestrategies such as the use of audit and peer review Maintain theirprofessional registration Record accurateconsultation data in patients records in accordance with the latest NMC guidance and other pertinent standards Keep up to datewith pertinent health-related policy and research. Critically appraise andsynthesise the work to underpin own practice and to inform others within thepractice team to consider the impact and strategies for implementation Workcollaboratively with colleagues within and external to the practice. Demonstrate teamleadership, resilience and determination, managing situations that areunfamiliar, complex or unpredictable and seeking to build confidence in others. Pro-activelypromote the role of allied professionalswithin the Practice, and externally to key stakeholders and agencies Act as an advisorto other members of the Primary Healthcare team. Proactivelyinitiate and develop effective relationships within the practice to encourageproductive working. Participate inmulti-disciplinary protocol and Patient Group Directions development Identifyappropriate opportunities to delegate both clinical and administrative tasks tomore junior staff. Help the practiceoperate in a cost-effective manner. Identify and managecare risks on a continuing basis. Participate inpractice meetings and practice management meetings, reporting progress asrequired. Evaluate ownpractice and participate in multi-disciplinary service and team evaluation.Actively engage in peer review to inform own and others practice, formulatingand implementing strategies to act on learning and make improvements. Work closely withothers in the setting up and/or improving of practice systems formonitoring/measuring performance against Clinical Governance and QualityIndicator targets. Ensure that allPractice policies are fully implemented Person Specification Experience Post registration experience to evidence autonomous clinical practice relevant to Urgent Care or General Practice Highly developed specialist knowledge underpinned by theory and experience Experience of working under pressure and managing varying workloads and changing demands Administration of medications within Patient Group Directions (PGDs) Sound knowledge of local primary care services including key players Demonstrable experience of CPD and desire to actively engage in ongoing learning of self and others. Demonstrable experience of advanced clinical knowledge in all age groups, without limitation in scope Experience of working within a multi-disciplinary team in an acute or Primary Care setting Able to evidence safe prescribing practice and is an Independent Prescriber Qualifications Registered General Nurse / Registered Paramedic autonomous clinical practice relevant to Urgent Care Further professional qualification e.g. ECP/ENP/nursing degree Masters Level Award covering and assessing all 4 pillars of advanced clinical practice (clinical practice, leadership and management, education and research) Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
