Advanced Clinical Practitioner / Frailty / Acute & Community role Band 8a - 23.63 hours per week Previous applicants need not apply NMC & AHP professional registrants can apply. An exciting opportunity has arisen within the Division of Intermediate Tier Services, rated OUTSTANDING by the CQC, we are seeking Advanced Clinical Practitioners to join our Acute Frailty Service, Community and Acute hospital work, within our new Acute Frailty Hub. This is a brand new service designed to provide high quality care to patients suffering from acute frailty syndromes. You will work across all Frailty services including Frailty Same Day Emergency Care, Drailty Decisions Unit, outreach and community. Role Overview Acute Frailty Hub is a multi-disciplinary team providing acute and community frailty service under Consultant Clinical Leadership. The post holder will be an autonomous Advanced Clinical Practitioner who will be expected to take an autonomous role in providing care and management. Responsibilities and Duties The successful candidate will be involved in the assessment, care and treatment of patients suffering acute frailty syndromes. The postholder will demonstrate specialist clinical skills and knowledge in advanced clinical assessment. The Advanced Clinical Practitioner will offer clinical expertise, guidance, and management plans, clinical decision making, and discharge planning to acutely unwell patients. They will develop differential diagnosis and work in collaboration with primary care, community services, acute trust and the wider multidisciplinary team to agree on what services need to be put in place, what clinical and medical support will be needed to manage and mitigate risk to ensure a safe and supportive care in their usual place of residence.
