Our service is dedicated to ensuring that individuals receive the support they need, especially during critical times outside of regular working hours. We are seeking a clinician to join our overnight service, providing care for palliative and end-of-life patients as well as catheter and urgent nursing care. This position entails working with an overnight team that manages referrals from 111 and is responsible for telephone triage, supporting base visits, and conducting home visits. We have dedicated cars within the bases with all essential equipment required for home visiting. The position offers learning and development opportunities, including access to a broad range of courses to enhance your clinical skills. This role is both challenging and rewarding, allowing you to apply your expertise to deliver care to a diverse patient population when at their most vulnerable. Experience in Palliative/end of life care and catheter care is essential for this role. Main duties of the job As part of a multidisciplinary team the post holder will work autonomously to provide a high-quality service to patients attending the Out of Hours Service (OOHs). They will foster excellent working relationships within the team. The post holder will triage, assess, treat, discharge, and refer to the most appropriate specialty or Registered Practitioner in order to meet the patient’s needs. They will support a learning environment that fosters a culture of clinical expertise and act as a role model to learners attached to the unit. They will work within the agreed clinical guidelines and the scope of professional practice.
