Full-time 37.5 hours a week - to be worked flexibly including weekends, bank holidays, evenings/nights. Qualifications We are not part of the NHS, therefore do not hold a licence to enable visa sponsorship. Successful candidate must hold a Nursing Associate Foundation Degree and work as an Associate Practitioner. Key Responsibilities Provide 24/7 Specialist Palliative/End of Life Care as part of a highly active community team. Provide care, advice and support to patients, their families and other healthcare professionals within the community. Work independently to complete full holistic patient assessments, ensuring needs are met in a timely manner. Liaise with the wider team to manage and Escalate any identified needs or concerns. Deliver palliative and end‑of‑life care as part of Mountbatten's multidisciplinary community services team, providing nursing interventions and ongoing assessment under the direct clinical leadership of the Clinical Nurse Specialists/Senior Nurse on Duty, reporting to the Community Team Leader.
