Community Palliative Care Staff Nurse

33 hours to be worked flexibly on a rota basis Monday to Sunday 8am – 8pm. We are proud to support and care for patients and their families at their most vulnerable time. We want to make it possible for more people who wish to stay at home to be cared for in familiar surroundings and with friends and family around them. Our Community Service includes Clinical Nurse Specialists, Registered Nurses and Multi Skilled Healthcare Assistants working together to deliver expert end of life and specialist palliative care to patients and families in their own home. We are looking for a dynamic, highly motivated, forward‑thinking registered nurse to join our community service as a Community Palliative Care Staff Nurse. Working alongside the Clinical Nurse Specialists and Multi Skilled Healthcare Assistants (MSHCA), you will take responsibility for a caseload of patients who require follow‑up care and rapid interventions at home. You will also have oversight of planned packages of care delivered by our team of MSHCA, supporting them to safely deliver personal care at home throughout our 7‑day service. Your clinical knowledge and assessment skills will be used to support patients who call for urgent assistance. You will work across the full week and weekends and be responsible for supporting the MSHCAs. Experience of working in the community or demonstrable understanding of the coordination and integration of community services is required. You must be able to work within a busy and close‑knit team whilst also demonstrating a degree of autonomy in your practice. You will be committed to person‑centred care and evidence‑based practice. We support continued professional development within the team, and you will be encouraged to seek out opportunities to learn and develop. We will provide a tailored induction and training programme to support your needs and integration into our service. You must be able to travel across our geography and be able to deliver bedside care. This post is subject to an Enhanced level check with the Disclosure and Barring Service.
