Overview Community Nurse – Band 5. Full‑time role within the Integrated Care team in Brentwood and Billericay. The team works in a fully integrated way with physiotherapists, occupational therapists, nurses, community matrons and care coordinators to provide a holistic approach to patient care. Responsibilities Assess, manage, plan and deliver care to patients allocated to them. Support peers and senior nursing staff and be an effective team member while working independently. Supervise pre‑registration students and Band 4 nursing assistants/assistant practitioners. Collaborate with other health and social professionals and agencies. Maintain accurate electronic patient records. Qualifications Registered Nurse with experience of working in the NHS. Community experience desirable but not essential. Induction and Probation New starters join the Trust on the first Monday of each month and undertake a comprehensive induction of up to two weeks, including mandatory training, systems training and equipment allocation. The induction is held at the head office in Rainham, Essex. All post holders are subject to a probationary period (internal applicants exempt unless they are currently part‑way through one).
