The service is an 8 bedded facility over two floors and located in a rural community. We provide a high level of support and encourage and support people to develop and maintain their individual skills. We also support and encourage people to access their community in order to support them to widen their social networks as well as promoting their skills in and around their home. We have an experienced and established team who are trained to support people's changing health needs as they get older. Communicate and work well with individuals to provide individual care and maintain appropriate relationships. Carry out tasks in accordance with organisational policies, procedures and guidelines. Being responsible in supporting individuals throughout the activities, giving positive feedback and encouragement.
