All Saints Medical Centre is a highly rated General Practice in Plumstead, South East London and we are looking for an experienced Practice Nurse who can offer a full range of nursing services to our 6k+ practice population. In addition to smears, children immunisations and contraception, we need someone with experience of either managing long term conditions or conducting annual reviews. This permanent position is available on a part‑time basis (2 days per week). Interest or experience in supervising trainee nurses is a plus but it is not essential. Main duties of the job Routine Practice Nursing e.g., Cytology (cervical smears) Child immunisations Management of long term conditions (e.g. diabetes) Contraception Dressings Phlebotomy (useful but not essential) Travel immunisations Any other practice nursing duties applicable to primary care About us Our practice team consists of GPs, GP registrars, Nurses, Clinical Pharmacist, Physician Associate, Physiotherapist, Advanced MHP, Live well coach, Social Prescriber, QOF Manager, Administrators and receptionists. We are Training Practice for GP registrars. CQC rating as Good. High QOF Achieving practice. Purpose built premises with Parking. Job responsibilities KEY AREAS OF RESPONSIBILITY Clinical Services and Caring for Patients Implement and evaluate treatment plans for patients with long-term conditions Long-term disease management, including (but not limited to): Asthma / COPD / Diabetes reviews Other chronic disease management/QOF activities Support and advise patients in managing their own disease Identify and manage treatment plans for patients at risk of long-term conditions Appropriately support patients in complex, urgent or emergency situations, and initiate emergency care as required Support and encourage patients to adopt a healthy lifestyle Give advice on prescribed or over-the-counter medications Support and advise women with family planning needs, including contraception and safe sex advice Carry out, support and advise women with regards to cervical smear tests Support and advise men with health/lifestyle advice and appropriate referrals Support and refer patients with mental health needs Carry out testing/screening, including (but not limited to): Urine testing/analysis Blood pressure measurement/monitoring ECG Height, weight measurement Take and test pathology samples from patients for analysis (blood, sputum, urine, etc.) Care for patients with uncomplicated wounds, including (but not limited to): Removal of sutures Tissue viability assessment Wound cleaning and dressing Management of burns and scalds Ulcer care, Management of minor injuries (sprains, bruising, cuts, etc.) Management of skin conditions and referral where necessary Run travel clinics and advise patients on vaccination, medication, malaria prophylaxis and avoidance, safe sex, food hygiene, health insurance, etc. Implement and participate in vaccination and immunisation programmes for both adults and children, including (but not limited to): Routine immunisations Pertussis immunisation for pregnant women Flu immunisation Pneumococcal immunisation Perform other clinical duties and Clinical Related Activities e.g. Ardens data quality lists checking as required, within the scope of the practice nurses competencies and training. Be prepared to carry out Home visits for the management of LTC for the Housebound patient cohort. No Job description can be entirely comprehensive. Qualifications Applicants must have current UK professional registration with NMC.
