The Urgent Care Team is a 24/7 service providing a first point ofcontact and comprehensive triage service, telephone and clinical assessment topatients and/or their carers/relatives resulting in a safe and appropriateclinical decision and plan of care.The Urgent Care Team is responsiblefor health and social care triage including supporting aversion of hospitaladmission or early hospital discharge through assessment and treatment ofindividuals in crisis, assessing complex and specialist needs in order todevelop, implement and evaluate programmes of care.
