The Urgent Care Team is a 24/7 service providing a first point ofcontact and comprehensive triage service, telephone and clinical assessment topatients and/or their carers/relatives resulting in a safe and appropriateclinical decision and plan of care.The Urgent Care Team is responsiblefor health and social care triage including supporting aversion of hospitaladmission or early hospital discharge through assessment and treatment ofindividuals in crisis, assessing complex and specialist needs in order todevelop, implement and evaluate programmes of care. Supporting individuals to remain in communitysettings, in their own home, residential home or nursing home for a shortperiod of time.If you are interested in joining us, you are compassionate,motivated and work well with others, then we would welcome your application.Please note that Care Plus Group are notcurrently registered to sponsor applicants from outside the EU and EEA.Main duties of the jobJob title: Urgent Care AssistantDepartment: Urgent Care TeamHours: 1 x 30 hours per weekPermanentSalary Range 3£24,968 pro rata per annum, fixed salary pointTheUrgent Care Assistant provides a short term crisis intervention service toindividuals within their own home environment that could include personal careand any support that has been identified through the assessment process.You will work in partnership with nurses, physiotherapists, occupational therapists or care managers across the Intermediate Tier Service and undertake delegated duties after appropriate instruction in conjunction with a detailed support/outcome plan.You will need to be able to work well asa team as well as using your own initiative and you will need to have goodcommunication and customer care skills.It is essential that you have 2years experience of working in a health and social care setting and haveknowledge of the principles of enablement and self-care principles.About usCare Plus Group is an employee owned Social Enterprise, establishedin 2011 to deliver NHS community health and social care services and employover 800 people.
