Band 5 - Community Staff Nurse – Discharge to Assess Stockport NHS Foundation Trust is seeking a Band 5 Community Staff Nurse to support our Discharge to Assess service. The role involves working with the Transfer of Care team and hub to triage referrals, provide holistic patient assessment, and support discharge planning. Job Summary The Community Staff Nurse works as part of the discharge services, completing and triaging referrals for Discharge to Assess, collaborating with hospitals, GP and social care services. Excellent communication and IT skills are required to assess and triage patients to the right team and services and to be proficient in the utilisation of virtual patient consultations. The role provides evidence-based nursing care and promotes self-care and independence. Main duties Undertake holistic patient assessment and triage appropriately. Implement and evaluate individual treatment plans for patients. Attend Whiteboard rounds to support discharge planning. Produce and maintain accurate written and electronic records in a timely manner, complying with information governance, confidentiality and data protection policies. Communicate with patients, carers and staff as part of effective patient care. Lead the team in the absence of a Band 6 or Team Lead. Facilitate professional development reviews with lower band staff after training. Act as assessor or supervisor for pre‑registration nursing students, trainee nurse associates/assistant practitioners and other learners. Act as preceptor for newly qualified registered general nurses. Take on a minimum of one clinical ‘link nurse’ role within the team. Person Specification Essential qualifications: Current 1st level registration. Desirable qualifications: Relevant post‑registration courses appropriate to the clinical area. Essential knowledge: Computer literacy; evidence of CPD. Essential experience: Experience of working in clinical areas; experience of delivering patient care. Desirable experience: Experience of supervising others. Essential skills: Effective time management; ability to work under pressure; resource management; teamwork; initiative; problem solving; motivation; adaptability; positive can‑do attitude; attention to detail; enthusiasm for results; patient dignity & respect; effective communication – written & verbal.
