Service Coordinator – Assisted Discharge Are you ready to lead a team that helps people move from hospital to home with dignity and care? As a Service Coordinator you will be the vital link between hospital referrals and care at home, managing a team of staff and volunteers to ensure services are delivered safely, efficiently and with heart. Location Antrim Area Hospital, Antrim, Northern Ireland Employment Details Salary: £23,590 to £25,155 per annum, pro‑rataHours: 21 per week – Saturday, Sunday, Monday 09:30 to 17:30Contract: Fixed term until 31 March 2026Driving Requirement: Full UK manual driving licence (minimum 2 years) and access to a vehicle Responsibilities Act as the main point of contact for hospital and community referrals. Coordinate and support a team of staff and volunteers to deliver services. Monitor service delivery, maintain accurate records and ensure quality standards. Attend hospital meetings, manage the service budget, and step in to support frontline delivery when needed. Qualifications Experience managing or supervising people and working in a team environment. Strong communication skills and ability to build relationships with health and social care professionals. Confidence using IT systems and managing service records.
