The Role As a Discharge Coordinator Nurse, you’ll support the Clinical Lead in the day-to-day running of the service, ensuring safe and effective patient care across community and hospital settings. You’ll deputise for the Clinical Lead when needed, help manage patient flow with the wider NHS team, and may be required to take part in the on‑call rota when necessary to support service continuity. Key Responsibilities Work closely with hospital staff to facilitate the effective flow and discharge of patients from hospital into our community teams. Conduct holistic clinical patient assessments and coordinate discharge plans, identifying and resolving internal discharge delays. Identify care needs and provide support and advice for all aspects of patient discharge. Review care requirements for potential patients to determine suitability for transition to the community setting, including virtual ward and early supported discharge pathways. Perform face‑to‑face reviews of patient medical history, liaise with the Trust’s MDT, and document clinical handovers in real time. Provide support, advice, information, and education to ward staff and MDT to facilitate a safe and timely discharge back to the community. Undertake all aspects of clinical duties as an autonomous practitioner. Educate patients and families, enabling them to make informed choices in discharge planning. What You'll Need Active NMC registration At least 3 years post‑registration experience as a registered nurse Previous experience in management or supervising staff is desirable Full UK driving licence and access to a vehicle Right to work in the UK (sponsorship not available) About You Recent experience with a broad range of conditions in adults Knowledge of the legal framework under the Care Act 2014 and duties relating to hospital discharge and other legislation relevant to discharge planning Right to work in the UK without any form of sponsorship Driver’s licence and access to personal vehicle Why Join HomeLink Healthcare?
