We operate a digital remote triage via EConsult designed to help as many patients as possible see the most appropriate clinician in the correct timescale. This role works with our experienced nursing team across two General Practice Sites on Monday, Tuesday, Wednesday and Friday.ResponsibilitiesCarry out Chronic Disease Management annual reviews delegated by a senior colleague.Support the Practice Nurse with the management of patients with long‑term conditions/chronic diseases, using a holistic patient‑centred approach.Assist in and perform routine tasks related to patient care as directed by the Practice Nurse, GP or other members of the Primary HealthCare Team.Enter baseline observations and identify signs of improvement; report any deteriorations or concerns (e.g. pulse, blood pressure, respirations, oxygen saturations, peak flow, spirometry, blood glucose levels, ECG, assessing nutrition and hydration).Participate in screening programmes and annual reviews for example in patients with Learning Disabilities.Perform cervical smear screening following additional training (RCN Position Statement on RNAs training in cervical sample taking, March 2021).Administer medications as prescribed and in accordance with NMC standards (e.g. via nebuliser or injections using sub‑cutaneous or intramuscular route under Patient Specific Directions).Assist in the assessment, planning, development, implementation and evaluation of programmes to promote health and well‑being and prevent adverse health effects.Deliver opportunistic health promotion during opportunities such as new patient medicals and NHS health checks.Document consultations accurately and appropriately, according to NMC guidelines.Ensure awareness of statutory and local safeguarding procedures, and recognise signs and symptoms of abuse/neglect.Maintain infection prevention and control guidelines.Provide appropriate nursing evidence‑based care for an allocated group of patients.Work as an accountable professional, recognising the limits of competency and working in line with NMC standards of proficiency.Collaborate effectively within the multidisciplinary team using resources efficiently.Deliver quality care to patients and work effectively within the nursing team.Person SpecificationEssential: Experience of working at NA level in general practice.Essential: Nursing Associate Foundation Degree (Level 5 qualification).Desirable: Child Immunisation Training.Desirable: Cytology Training.RequirementsDisclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and will require a DBS check.UK Registration: Applicants must have current UK professional registration.Employment DetailsSalary: Depending on experience.Contract: Permanent.Working pattern: Full‑time, Flexible working.Reference number: A1607-NADec2025.EmployerHeaton Medical Centre2 Lucy Street, Bolton, BL1 5PULocation2 Lucy Street, Bolton, BL1 5PUAvondale Health Centre, Avondale Street, Bolton, BL1 4JP #J-18808-Ljbffr
