The Inpatient Haematology Ward is pleased to announce the opportunity for a dynamic, organised and proactive Band 3 Discharge co‑ordinator to join our team. As the successful post holder you will work closely with the nursing staff and wider MDT including Discharge Team, Palliative Care Team, Social Services, Care Agencies and Care/Nursing Homes to facilitate the safe and effective discharge of our patients. You will gain experience in managing complex discharges, safeguarding, dealing with challenging situations and communicating with outside agencies. The post comes with full on the job training with an induction to the hospital and the role included. Person Specification Experience Experience of working in a hospital setting or relevant community service Understanding of complex discharge planning in care of adults and vulnerable individuals. Experience of supporting patients' specialised care needs to enable informed decision making Previous experience of working with patients requiring CHC and Social care involvement. Qualifications Good standard of education NVQ level 3: health / social care management or equivalent Assessment, planning and organisational skills Ability to progress plans for discharge with internal / external agencies and family members Able to intergrate and work within the MDT Time Management Skills Use of IT including Windows, Word, Excel, Powerpoint, ECAMIS, ENA, Good verbal communication skills both face to face and over the telephone. Use of EPR, Mosaiq, eforms systems Knowledge Community referral processes Knowledge of the complex discharge planning process. Understanding of assessing and problem solving issues related to discharge planning Understanding of multidisciplinary assessment and treatment for adults/ vulnerable individuals within acute care and community settings Understanding of the new Care Act, NSF's, CHC Framework, Delayed Discharged Act and Reimbursement Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
