Band 6 UCR/VW Nurse The closing date is 22 December 2025 About the role The Urgent Community Response Team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re‑engage with the important aspects of their life and prevent admission to acute sectors. The post holder will work in the Urgent Community Response Team and Virtual Wards to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service. The post holder will ensure access to urgent health assessments, provide and monitor short‑term treatment programmes at home and ensure safe discharge or referral to long‑term health and social care interventions. They will use clinical expertise, judgement, discretion and decision‑making to develop individualised treatment plans. They will work in partnership with primary, secondary and other health and social care providers to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, while improving the quality of care. They will put the patient at the centre of all decisions, reinforcing a more streamlined patient journey and improving the quality of care. The post holder will be expected to work within a flexible working pattern over a 7‑day week in the urgent community response team as required. Main duties of the job Participate in the development and implementation of local nursing/therapy strategies and policies for patients on a UCR pathway or on a virtual ward Act as an expert practitioner, professional role model and specialist resource to the multi‑disciplinary team regarding UCR/Virtual Ward Actively promote participation by the team in current quality initiatives Manage own time effectively and prioritise workload Facilitate teaching and supervision for less experienced members of staff and students >Promote health by collaborative working with patients and their relatives Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately Participate in the further development of an integrated, multi‑disciplinary Urgent Community Response Service working alongside colleagues in integrated multidisciplinary teams and the wider health and social care economy Keep accurate, up‑to‑date clinical records and ensure timely inputting of relevant activity data About us University Hospitals Birmingham NHS Foundation Trust strives to have an inclusive culture where everyone feels like they belong, can thrive, knows that they add value, and feels valued. Person Specification *Please Note : For a detailed job description for this vacancy, please see attached Job Description* Qualifications Relevant clinical qualification with current registration (e.g. RGN) Relevant Degree level qualification or equivalent experience Minimal 4 years post‑registration experience Evidence of continuing professional development Health assessment course Non‑medical prescriber (if applicable) Experience Experience of working in a multi‑disciplinary team setting Experience of comprehensive clinical assessment of patients Experience of caring for patients who are acutely ill/require rehabilitation plan/end‑of‑life care Experience of effective caseload management Experience in line management and supervising junior staff Experience in facilitating professional development of other staff and ability to motivate others Experience in collection, collation and analysis of data for clinical audit and research Additional Criteria Professional appearance Displays genuine interest in the post applied for and can articulate this when questioned Positive and enthusiastic attitude Ability to communicate articulately with others Caring and approachable Punctual Reliable Ability to work a varied shift pattern Unsocial hours where required Ability to describe why the Trust may deploy you to a different practice setting for a span of duty Able to meet the travel requirements of the post and be independently mobile; possessing a full category B driving licence, preferably clean or with no more than a maximum of 3 points Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
