Physical Healthcare Manager – Low Secure & Community Forensic Care An exciting opportunity for a Physical Healthcare Manager within the Low Secure and Community Forensic Care Unit at the Wells Road Centre, In-patient unit. Role Overview The post holder will assume a key role within the in-patient multi-disciplinary team to provide expert clinical case management, advice and support to in-patients with long term or deteriorating physical health, aiming to improve quality of life and prevent hospital admission. Responsibilities Conduct high level assessment, contribute to differential diagnosis and lead to definitive diagnosis to initiate medical treatment in conjunction with medics. Lead coordination of care working with partner agencies and key stakeholders. Develop and implement appropriate pathways of care for patients with long term physical health conditions. Provide clinical and managerial leadership to promote best practice and develop Physical Health services to enhance patient experience. Assess, review and evaluate needs of patients and carers to improve physical and psychological well-being. Work in partnership with patients, carers, consultants, medics and other professionals to instigate diagnostic testing and therapeutic treatments. Use multi-disciplinary ward rounds, systematic reasoning and justified conclusions for diagnosis and proactive care plans. Practice as an independent prescriber, apply knowledge of medicines and prescribing skills to reduce risk and complications. Promote self‑care, independence and recovery within the team and patient group. Requirements Qualified medical practitioner with experience in physical health care within forensic or secure settings. Experience in independent prescribing and multidisciplinary teamwork. Strong clinical assessment and diagnostic skills, with evidence‑based practice. Excellent communication, leadership and self‑management skills. Additional Information Applicants will be required to pay for their DBS check.
