We are looking for an experienced part-time Practice Nurse to join our friendly and supportive team at Parkway Medical Group. ThePractice is committed to delivering high quality patient care which requires usto have a supportive and cohesive team.We are seeking a skilled and enthusiastic Practice Nurse with chronic disease experience to join ournursing team. Due to room constraints the days of this post is none negotiable.Hours:Thursday 8am - 6pmFriday 8am - 6pmMain duties of the jobWe are seeking an experienced Practice Nurse who will fulfil core responsibilities of the role which will include: Long Term Condition management, wound care management, Cytology, Child Immunisations, Vaccinations Contraception, Swabs, specimens, bloods, blood pressures, infection control, keeping accurate and detailed health records on SystmOne clinical system. A key role in QOF/PEP achievement, Involvement in the MDT of the Surgery. This role may support the wider team with: Clinical audits, Safeguarding concerns, Support and participate in shared learning over the Practice Support for GPs in patient care. Ability to review clinical practices and respond to national policies and initiatives where necessary. Participate in local initiatives in order to enhance local patient care where necessary.About usParkway Medical Group is a friendly, well-established, enthusiastic practice with around 8,350 patients. We have 3 GP partners, 5 salaried GPs, 2 Practice Nurses and 2 Health Care Assistants. The practice is based in a modern purpose built Health Centre in Chapel House.High QOF achievement year on yearRegular training sessions throughout the year.Nurse and HCA team have a daily morning coffee break.Weekly Clinical team huddlesWeekly Nurse/HCA meetings and weekly clinical meetings.We use SystmOne Clinical system.Job responsibilitiesMain Duties and responsibilities:Wound careRemoval of Sutures / clipsUrinalysisECGsYear of Care and Chronic disease Management such as Diabetes, Asthma, COPD, Hypertension,VenepunctureCervical cytology smear testsHealth ScreeningGeneral health adviceRoutine immunisations / Child ImmunisationsRoutine InjectionsContraceptionTravel ClinicChaperoning and assisting patients where appropriate who are being examined by another clinicianRequesting pathology tests, for example urine culture, swabsFollowing agreed clinical protocols with referral to other nurses or GPs as appropriateMaintaining and cleaning equipment used by the nurses and GPsParticipate in maintaining Nurses rooms, stocking and rotating items as requiredUndertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms keeping a clear work surface cultureOrdering of stock to maintain safe stock levels & cash controlOrdering of vaccinations to maintain safe stock levels & cash controlAt all times there will be a need to maintain accurate and contemporaneous recordsA duty to advise Practice manager and Partners of potential problems or errors within the range of assigned tasksAttend and participate in any Practice meetings when requiredAny other delegated duties appropriate to the postFlexibility of working to ensure sufficient cover is provided for nurse holidays/sicknessMaintain effective liaison with other agencies & staff concerned with patient careCo-operate & participate as required in any research projects within the practicePerson SpecificationQualificationsRGN Experience of working in Primary CareUnderstanding the complexities of managing changeManagement and education of patients with chronic diseases Childhood immunisationsCytology and womens health.Travel healthSexual healthSpirometry Ability to work on own initiativeExperienceMinimum 2 years post registration experienceAt least 1 year recent primary nursing experienceManagement of long-term conditionsInvolvement in implementing and using protocols and clinical guidelinesCQC experience of requirementsExperience of auditOther requirementsWilling to work flexible hours as necessaryHighly motivatedEnthusiastic Team playerBasic IT SkillsDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
