As partof the Hounslow Health Primary Care Network, we collaborate closely withpartner practices to deliver innovative and integrated healthcare.We currently serve a list ofover 14,000 patients with a team that includes GPs, nurses, pharmacists,healthcare assistants, physician associates, social prescribers, and wellbeingcoaches. Our environment promotes learning, development, and collaborativepractice.Job responsibilitiesThe main duties include but are not limited to:Administering vaccinations and immunisations in line with national and practice guidelinesCervical cytologyManaging and monitoring patients with chronic diseases (e.g. asthma, COPD, diabetes, hypertension) in line with national and localguidelinesCarrying out home visits for housebound patientsSupporting the clinical team in the management of the carehomesEnsuring accurate and timely clinical record keeping and documentationConducting contraception reviews and coil checksCarrying new patient and NHS health checksWound care and management including dressing, removal of sutures and clipsParticipating in infection control, cold chain maintenance, ensuring fridge temperatures are regularly monitored and maintainedSupporting patients with lifestyle advice and self-caremanagementAdvising patients on health care, minor ailments, healthpromotion, and illness preventionContributing to achievement of QOF (Quality and Outcomes Framework) and local enhanced services achievement and clinical auditsPreparing and maintaining clinical rooms and equipment before,during and after patient care activities, ensuring a clean and safeenvironmentEnsuring emergency medication and resuscitation equipment is adequately stocked andfit for purposeEnsuring proper handling and dispatch of laboratory specimens including blood and urine samplesParticipating in practice meetings and contributing to our aimfor continuous improvementSupporting the training and mentoring of junior staff members and HCAsPerson SpecificationExperienceExperience of working within a nursing team in primary careExcellent communication and interpersonal skills, and be confident to deal with patients, families and carersAbility to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the positionCommitment to patient confidentiality and data protectionAccuracy and a strong desire for attention to detailCommitment to continuing professional developmentWorking knowledge of SystmOne clinical systemStable career history showing clear commitment and progressionQualificationsRegistered General Nurse with current NMC registrationQualified & competent in cervical cytology, immunisations, and chronic disease management and phlebotomyDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
