Advanced Pharmacist – Acute Care (QEH) You will be an Advanced Pharmacist in the Acute Care wards, supporting the development of the clinical pharmacy service across all acute care areas, including the Emergency Department, Medical and Surgical Admissions Units, Short Stay Unit and Ambulatory Emergency Care Unit. You will ensure the safe, clinical and cost‑effective use of medicines and staff resources in line with local and national standards and policies. Responsibilities include: Provide highly specialised pharmaceutical care to patients and support other healthcare staff in meeting risk management, financial management and clinical governance responsibilities around the use of medicines. Advise pharmacists and healthcare professionals and participate in multidisciplinary and governance meetings. Deliver departmental training and provide clinical supervision and mentoring for junior pharmacists and medicines management technicians. Support the quality training experience for trainee pharmacists, pharmacy students and team trainees. Carry out clinical pharmacy audits and practice research in collaboration with the Lead pharmacist. Lead and coordinate the clinical pharmacy service in Acute Care areas, visiting wards, speaking with patients to review their medicines and ensuring prescriptions are appropriate and safe. Practice medicine optimisation during daily ward rounds, determining the most appropriate choice of medication in response to complex data and reducing avoidable adverse events. Work with medical, nursing and site management teams to facilitate patient discharges and maximise patient flow.
