They participate as a member of the physiotherapy team, independently managing a specialist clinical caseload with the Monmouth Community. The post holder will be part of the wider Older adult Integrated Team at Monnow Vale Health and Social Care Facility, working alongside Health and Social Care colleagues. They take responsibility for supervising, delegation, training and developing others. The post holder will contribute to implementing changes in their own clinical area to improve patient care and efficiency of service delivery. Reporting to: Lead Physiotherapist Operational Manager Accountable: Operational Manager Professionally: Head of Physiotherapy Service Main duties of the job Conduct holistic physiotherapy assessments and provide specialist, client-centred interventions in patients' homes and other community settings. Work autonomously within the multidisciplinary and Integrated team to develop and implement personalised treatment plans, aiming to maximise patient independence and prevent hospital admission. Supervise junior staff and students, collaborate with other health and social care professionals, and manage a caseload of complex and varied needs. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply. Assess patients’ capacity, gain valid informed consent to treatment, and where such capacity is lacking or absent, work within a legal framework in the management of the patient. Undertake comprehensive assessment and accurate diagnosis of patients, including those with a complex presentation, using investigative, palpatory, analytical and clinical reasoning skills. Implement specialist physiotherapy treatments/programmes. Utilise highly developed manual skills in physical treatment techniques. Formulate individualised clinical management programmes, using a wide range of treatment skills and options to plan a specialised programme of care. Provide spontaneous and planned advice, teaching and instruction to relatives, carers, other disciplines and agencies to promote understanding of the aims of physiotherapy and ensure continuation of the treatment programme. Participate in and, where indicated, initiate multidisciplinary/multi‑agency team meetings and case conferences to ensure the coordination of patient care. This may include the review of patient progress and discharge planning. Ensure accurate, comprehensive and up to date clinical records are maintained in accordance with Health Board guidance and professionally agreed criteria. Produce comprehensive patient‑related reports for other disciplines or agencies relating to assessment findings and/or treatment outcomes. Work to Health Board, Professional and National clinical guidelines and have a good working knowledge of relevant national standards for quality practice. Assist the associate Team Lead/Physiotherapy Manager(s) with the prioritisation and management of a designated departmental patient caseload. Communicate effectively with patients, relatives, colleagues and professionals. Deputise for senior staff where appropriate. Participate in the Health Board individual performance review process as both appraiser and appraisee. Assist in recruitment and selection procedures for the Physiotherapy Service. Contribute to the induction of physiotherapy staff. Participate in and, if requested, lead departmental meetings to ensure the effective exchange of information across the team and coordination of local service delivery. Take responsibility for the safe use of equipment and advise appropriate personnel of the need to review availability of suitable resources for patient care. Adhere to the policies and procedures of the Health Board and Physiotherapy Directorate. Maintain state registration with the Health Professions Council (HPC) and provide evidence of that registration. Participate in the Health Board’s individual personal review process and take responsibility for the adherence to an agreed personal development plan. Be responsible for maintaining own competency to practice through continuing professional development activities including reflective practice, review of relevant literature, participation in local in‑service training programmes and peer review, maintenance of a personal portfolio, and attendance of relevant training courses as identified within a personal development plan. Be an active member and contribute to the organisation and delivery of the in‑service training programme. Take responsibility for the tuition of Specialist and General Rotational Band 6 and Band 5 physiotherapists and physiotherapy support workers, other as identified through the personal review process. Complete a clinical educator course and undertake the tuition, supervision and performance assessment of undergraduate physiotherapy students, working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification. Provide teaching and instruction to other disciplines to ensure a consistent approach to patient care. Participate in mandatory/statutory training as outlined by the Health Board.
