We have a new and exciting opportunity for Clinical Case Managers to join our Complex Transition Support Service (CTSS)Due to the nature of this role we require a driver with access to their own vehicle who is happy to travel.This is a hybrid role, with an expectation to travel across the locality to complete visits to wards and community providers.The service covers the full locality of the North East and North Cumbria, however you would largely be working within a specific geographical area.The service will work closely with North East and North Cumbria (NENC) Integrated Care Board (IBC) Complex Case Management team. The overall aim for CTSS is to support the discharge of people from acute secondary care inpatient beds / private hospital beds into alternative community settings. CTSS will also assist in the repatriation of people back to the NENC region for those currently residing out of area. The service will provide assessments, discharge planning support and 12 weeks post transition support to settle people into their new accommodation.We are keen to here from Registered Mental Health Nurses (RMN), Registered Learning Disability Nurses (RNLD), Occupational Therapists (OT) and Social Workers who have experience working with complex mental health needs, autism and learning disabilitiesAbout the jobAs our Clinical Case Manager, you will be responsible for assisting the Senior Clinical Case Managers in organising and developing service provision, including staffing, budget allocation and expenditure, resources, support networks and public relations within and relating to the service.Key responsibilitiesManage the needs of service users, carers and partner services to ensure delivery (inclusive of all required resident records) meets all relevant quality standards and is person centred, outcomes focused and protective of individual service users’ dignity and safety.Manage a team of up to 4 staff ensuring the effective and efficient deployment of resources to achieve agreed outcomes and targets.Coach and Mentor student nurses and newly qualified nurses to support with individual development.Provide auditing of the service on a regular basis as and when required for senior management and governance teams.Undertake any reasonable duties/responsibilities to meet the needs of the organisation.Maintain positive relationships with ICB Case Managers, Local Authority Placement Coordinators, NHS Trust Bed Coordinators.Carry out assessments, facilitate discharge plans, create and support the 12 week transition plan when the service user moves into their new accommodation.Provide information for funding panels to support effective move on for individuals.About youRegistered Nurse or Registered Allied Professional.Significant experience of working with people with complex mental health needs.Demonstrable experience of meeting agreed performance targets in a service demonstrating clinical outcomesDemonstrable experience of working with people who have experienced common mental health problems i.e. anxiety / depressionDemonstrable experience in robust risk management processesAdvanced understanding of complex mental health conditions and behaviours relating to or equivalent experienceUnderstanding of local and national policies on mental health service provisionUnderstanding of laws, mental capacity act, Mental Health Act, SafeguardingWhat we offer in returnWe are proud to have been recognised and certified as a Great Place to Work, which speaks volumes on how much we value our staff members.
