The Harness North PCN ClinicalPharmacist will be joining a dynamic and innovative Clinical Pharmacy team whichhas been established for several years and includes trainee pharmacists andpharmacy technicians. This is an excellent opportunity to be part of a valuedteam within the Harness network.Main duties of the jobThe Harness NorthPCN Clinical Pharmacist will be supporting the PCN and practices to deliverquality improvement and practice deliverables. This will include but is notexhaustive to; population health management via long term condition reviews,SMRs and audits, medicines optimisation and reducing medicines waste.They will provideexpertise in clinical pharmacy as part of a multi- disciplinary team supportingin the delivery of population health management for the PCN and clinicalmanagement of patients within General Practice and community sites, includingcare homes and patients homes. Experience of working in a PCN is essential.About usHarness CareLtd provides general and extended medical services to a population of 140,000patients residing across Brent in Neasden, Harlesden, Wembley, Willesden andKingsbury. Our 20 GP practices work across two Primary Care Networks (PCNs).Harness North PCN provides general and extendedmedical services to patients residing across Neasden, Wembley and Kingsbury.Harness North PCN consists of a team of 10 practicesJob responsibilitiesClinicalresponsibilities include a variety of mediums for patient contact, engagementand support:Provision of professional support and clinicalexpertise to the PCN and wider teams with regards to medication queries,prescribing issues and systems for safer prescribing.Supervision and support of the CP team.Oversight on key PCN deliverables.Face to face/remote long term condition reviews,Structured Medication Reviews including Care Home Reviews.Lead delivery of quality improvement, populationhealth management and clinical audit for PCN in pharmacy led clinical domainssuch as Impact and Investment fund (IIF), Medicines Management (MM) in the PCNDes and CQC compliance.To support practice teams with day-to-daypractice such as medicines reconciliation, repeat prescribing practice andsupport with queries from patients and teams whilst utilising digital tools tosupport with this such as e-consults, Docman and internal tasks.Support key stakeholders with streamlining theCP service.Working closely with the Clinical Directors andkey stakeholders to shape and deliver the strategy and vision for the PCN.KeyPriorities for the roleMake the Clinical Pharmacist a principle in thedelivery of high-quality, patient-centred multidisciplinary care.Utilise the Clinical Pharmacist skill set toallow General Practitioner appointments to be reserved for appropriatecaseload, reduce waiting times for appointments, increase access to healthcare,improve screening and diagnosis of chronic and common ailments and reduceA&E admissions and attendances.Provide direct care to patients with long-termconditions, minor ailments and other clinical areas within scope of practice.Provide professional input to practice teamsworking to improve patient outcomes and reduce avoidable admission to hospital.Liaise with providers including pharmacists inthe community and hospital settings to ensure accurate and effective medication management.Provide pharmaceutical support to practices sothat medication related processes are reviewed, best practice principlesapplied and evidence-based values are embedded in all activities.Utilise the Clinical Pharmacist skill set todirectly assist patients in the safe, effective use of medicines to improveoutcomes and quality of life.Ensure optimum benefit for prescribed medicationthrough high levels of concordance achieved by support for patients and carers.Support the care of complex patients in theirown homes or in nursing or residential accommodation as part of amultidisciplinary team.Utilise the Clinical Pharmacist skill set toensure NHS resources are used in a cost-effective manner to maximise outcomeswhilst reducing wastage.Provide professional leadership on mattersrelating to medicine optimisation and quality improvement.Person SpecificationQualificationsPharmacy DegreeRegistration with the General Pharmaceutical Council (GPhC)Centre for Pharmacy Postgraduate Education primary care Pharmacy Education Pathway (PCPEP)Registered non‑medical prescriberPostgraduate diploma in Clinical Pharmacy or equivalentExperienceAt least 12 months experience working in a PCN/practiceExperience of working with clinical management systems, especially Emis & System OneRelevant theoretical and practical knowledge of Primary Care Networks, General Practice and evidence-based medicineAn appreciation of NHS agenda, national policies and government targets and quality improvement indicatorsAn understanding of the current PCN DES specificationsA wide range of IT skills especially Microsoft 365 softwareAwareness of business intelligence systems to support management of patients in primary care settings, delivering pharmaceutical input and support in the context of pathways of care and the business of the organisationAbility to work under pressure and deal with unpredictable issues arising on a daily basisCan plan, manage, monitor, advise and prescribe medication for patients in core areas, including long term conditions.Be able to organise workload, multi-task and work under pressure to achieve tight deadlinesAwareness of PCN budget management and funding systemsRelationshipsWork autonomously and within a teamThe post-holder will support the equality, diversity and rights of patients, carers and colleaguesTraining practice staff to ensure efficient prescribing and medicines management proceduresAbility to influence, motivate and persuade prescribers to comply with the recommendations or agreed course of action where there may be significant barriersAbility to lead on quality improvement work stream(s) and delivering quality outcomesAbility to support Foundation, Entry, Trainee pharmacists and Pharmacy Technicians with their development and day to day practiceAbility to support Senior CP team and proactively seeking innovative ways to improve prescribing practice and PCN developmentFlexible approach to working patterns and ability to adapt to changing prioritiesSelf motivated and proactiveAbility to provide education and training to PCN teamsCommunication & InformationExcellent interpersonal and communication skills, both written and verbal.Clear, concise record keeping and report writing skillsDemonstrate the ability to communicate complex and sensitive information in an understandable form
