Overview37.5 hours per week. Doncaster Crisis Team: a role involving triage assessment and intervention for service users in crisis, including telephone contact, home visits and assessments in A&E. A permanent Band 6 qualified practitioner is sought to join a busy team, with a focus on delivering a caring and safe service in the community.We are looking for dynamic, positive practitioners who work within a strengths and recovery based approach and who are passionate about supporting individuals in crisis.ResponsibilitiesProvide triage assessment and intervention for service users in crisis.Conduct telephone contacts, home visits and assessments in A&E as part of crisis management.Liaise with families to support their understanding of crisis management.Work with partner organisations, third sector and peer support services to deliver coordinated care.Contribute to and lead changes as part of the community working age model and crisis service development.Qualifications and RequirementsPermanent Band 6 qualified practitioner.Enthusiastic, highly motivated and reliable to work in a busy crisis service.Passion and desire to work with individuals in crisis within the community, delivering a caring and safe service.Progressive, strengths- and recovery-based approach.Ability to liaise with families and partner organisations in a transparent and open manner.Additional informationRotherham Doncaster and South Humber NHS Foundation Trust (RDaSH) employs around 3,700 colleagues across mental health, physical health, learning disability and drug and alcohol services.
