Pay Rate: £12.75 per hour (PAYE) / £16.35 per hour (Umbrella)
About the Role
An exciting opportunity has arisen within the Calderdale Pathways Service to support care leavers aged 18–25 as they transition from care into adulthood. This post has been approved to provide additional capacity in response to increased service user demand.
The role is central to delivering Calderdale Council’s responsibilities under the Children (Leaving Care) Act 2000, ensuring young people receive high-quality, child-centred support aligned with the council’s Systemic Practice Model and ethos:
“Would this be good enough for our own child?”
You will act as a Corporate Parent, working directly with care leavers to help them achieve positive outcomes, independence, and long-term stability, while benefiting from Calderdale’s strong commitment to staff development and high professional standards
Key Responsibilities
Manage a caseload of care leavers aged 18–25 (statutory support up to 21, extended support up to 25).
Act as a Pathway Advisor, providing advice, guidance, assistance, and practical support.
Co-produce, review, and deliver high-quality Pathway Plans to support young people’s goals and aspirations.
Work in partnership with young people’s networks, including families, friends, and statutory and voluntary agencies.
Coordinate and facilitate group work and enrichment activities with young people and their networks.
Practice within a multi-disciplinary, systemic framework, contributing to cohesive, child-centred support.
Maintain accurate, timely records and reports in line with practice standards and performance requirements.
Participate in duty systems, supervision, team meetings, and service development activities.
Contribute to reflective practice, constructive challenge, and continuous service improvement.
Represent the service in line with Calderdale Council’s values, equality commitments, and professional standardsExperience & Knowledge
Essential:
Relevant experience working with young people and/or young adults, particularly those at risk of social or educational exclusion.
Experience of caseload management and associated administrative responsibilities.
Knowledge of relevant legislation, policy, and guidance relating to children, care leavers, and young people.
Understanding of safeguarding requirements and ability to make sound professional judgements.
Experience of multi-agency and partnership working.Desirable:
Experience of systemic practice.
Social Work qualificationSkills & Competencies
Ability to complete robust assessments, risk assessments, and support plans.
Strong written skills, with the ability to produce clear, high-quality reports.
Excellent communication and interpersonal skills, including relationship-based practice.
Strong organisational skills with the ability to manage competing priorities and meet deadlines.
Commitment to equality, diversity, and inclusion.
Ability to work independently while contributing effectively to a team environment.Additional Requirements
Level 3 qualification or above in a relevant field (essential).
Full UK driving licence and access to own vehicle with appropriate business insurance.
Willingness to work flexibly, including occasional evening or out-of-hours work.
Enhanced DBS check, including Children’s Barred List.
Commitment to safeguarding and promoting the welfare of children, young people, and vulnerable adults.
