We are looking for an experienced and motivated Physiotherapist to join our multidisciplinary Primary Care Home team in Lowestoft. You will provide holistic physiotherapy assessments and interventions in patients’ homes, supporting rehabilitation, preventing unnecessary hospital admissions, and promoting health and well‑being. About The Role You will manage a varied caseload of adults in the community, delivering physiotherapy interventions to support independence and recovery. This includes: Crisis assessments and rapid response Rehabilitation and reablement Preventing unnecessary hospital admissions You will also provide clinical leadership and line management for other members of the multidisciplinary team. Key Responsibilities Deliver physiotherapy assessments and interventions in patients’ homes and community settings. Manage a caseload of patients with acute, chronic, and age‑related conditions. Treat and manage a variety of presentations and conditions working alongside specialist services. Participate in rapid response and crisis assessments to prevent hospital admissions. Work autonomously within scope of practice and professional standards. Provide clinical leadership, operational management and professional support to the wider team. Contribute to recruitment, retention, and team development. Use SystmOne for clinical documentation and mobile working (training provided). Legally accountable for all aspects of own work, including patient management. Responsible for equipment used, ensuring safe use and providing supervision. Provide comprehensive assessment of patients, including complex presentations. Formulate individualized physiotherapy management plans using a range of treatment skills. Provide advice, teaching and instruction to relatives, carers and professionals. Communicate effectively with patients and carers, ensuring understanding and consent. Assess capacity, gain valid informed consent and work within a legal framework for patients lacking capacity. Professional Maintain own CPD, attending training and presenting at staff meetings. Maintain accurate, comprehensive documentation and report to appropriate disciplines. Collaborate with medical, nursing and therapy colleagues for coordinated service delivery. Train, supervise and perform performance management of junior staff with senior guidance. Participate in staff appraisal scheme and Personal Development Programme. Undertake evidence‑based practice projects, audits, and outcome measures. Demonstrate understanding of Clinical Governance and Risk Management. Organisational Deputise for the Band 7/Team Leader in operational matters. Collect appropriate data and statistics for the department. Implement health and safety policies and report incidents promptly. Review and update departmental policies and procedures. Perform other duties appropriate to Band 7/Team Leader or Professional Head of Physiotherapy Services. Person Specification: Personal Attributes (Essential) Physical capacity for moderate effort daily and the ability to deal sensitively with anxious/aggressive patients. Capacity to concentrate for long periods and respond immediately to interruptions. Collaborate within a team and work independently. Committed to personal and team development. Maintain judgement, motivation, enthusiasm and confidence under pressure. Desire to work in a clinical area commensurate with the post. Flexible working and ability to travel across relevant areas. Person Specification: Personal Attributes (Desirable) Able to recognise own limitations while identifying learning and development needs. Experience Essential Relevant postgraduate experience. Desirable Experience at Band 6 level in the community setting and/or acute rotational setting. Supervisory skills. Skills and Knowledge Essential Good knowledge of physiotherapy in relevant area. Work in a busy environment managing patients with complex health needs. Forward thinking and excellent interpersonal skills. Self‑directed learning and ability to share knowledge with others. Desirable Excellent knowledge, critical appraisal, audit, research, presentation skills. Neurology, respiratory and orthopaedic specific postgraduate training. Qualifications Essential Diploma / Degree in Physiotherapy. Registration with HPC. Evidence of relevant postgraduate courses / CPD. Evidence of continued professional development including management/leadership development and a professional portfolio. Desirable Relevant validated clinical courses.
