The post holder will be based in the Occupational Therapy service, Mental Health, Coity Clinic, Princess of Wales Hospital. The post holder will deliver evidence based, recovery focused Occupational Therapy specialist assessment and interventions to service users accessing facilities across unscheduled care (Home Treatment Team, Ward 14 and the Psychiatric Intensive Care Unit). The post holder will be affiliated with Ward 14 but work within an Occupational Therapy whole team approach to ensure service delivery across unscheduled care. Thus providing continuity of care to service users through their recovery journey and the opportunity to complete the OT process from admission to discharge. The post-holder will be supported by a Band 7 Occupational Therapist, work alongside a further band 6 Occupational Therapist and support a rotational Band 5 Occupational Therapist. The post holder will be responsible for delivering Recovery focussed individual and group Based Occupational Therapy sessions. All Occupational Therapy assessments and interventions will be evidence based, and evaluated using recognised outcome measures; assessments and interventions delivered will be measured against a range of performance indicators. Main duties of the job Manage a defined caseload, providing specialist OT assessment and appropriate evidence based intervention, utilising a recovery focussed model, with individuals referred to the service - promoting independence, recovery and mental well-being within hospital, service users' homes, and community venues. Use the Occupational Therapy process autonomously in various settings, including lone working in the community and in service users' homes across Cwm Taf Morgannwg. Deliver assessment and intervention inputs using validated and evidence based models of care, assessment tools and outcome measures - such as the Model of Human Occupation. Participate in formal meetings, providing expert witness/specialist knowledge of occupational and mental health perspectives. Work effectively with multidisciplinary (MDT) colleagues to ensure robust therapeutic environs for service users which support autonomy, maintain function and productivity throughout their inpatient stay. Contribute effectively to home-leave planning, articulating risk information and opportunities to MDT, and supporting service users to accessing productive and meaningful periods of home leave.
