Social Work Advanced Practitioner – Adults Hospital Team We have an exciting opportunity for an Advanced Practitioner within Adults Hospital Team in Barnsley, earning £37 per hour, hourly. This is an ongoing contract for a full-time position. You will work in collaboration with health professionals across various discharge pathways, ensuring a seamless and swift flow for patient transitions. As a pivotal member of the team, you will utilise your prior experience at an Advanced Practitioner level to support and lead social care staff with a deep understanding of legislative frameworks and complex case management. What you will do: Collaborate with another Advanced Practitioner and the Team Manager to oversee a team of 22 Social Workers and Assistant Social Care Practitioners. Manage and monitor daily operations, including dealing with referrals, assigning tasks, and responding to staff inquiries. Attend daily meetings with health partners and ensure timely supervision of staff. Audit and authorise Care Act assessments and support plans, ensuring compliance and quality. Manage care resources and budgets effectively, ensuring financial efficiency. Maintain a small, manageable caseload dealing with individuals with multifaceted health and social care needs.
