About the Role Are you a caring individual looking with previous background working in a mental health environment? Our Blandford Older Persons Community Mental Health Team would love for you to join their team Being a driver is essential for this role. Key Responsibilities Support adults over the age of 65 with mental health conditions as part of a multi-disciplinary team. Develop holistic individualised care plans with qualified staff and other care networks. Assess changes in service users on visits, including use of formal testing scales, identifying early indicators of relapse and communicating alerts to appropriate staff. Make decisions using own initiative while recognising limitations and knowing when to refer to other agencies. Follow up with service users regularly, developing rapport and providing information about health and wellbeing, identifying barriers to accessing services or groups and facilitating solutions. Working Hours & Location This is a full‑time role Monday to Friday 09:00-17:00 (flexible start and finish times). Base: Blandford Community Hospital, covering the surrounding areas. Qualifications & Skills Strong communicator, organised and confident working alone. Ability to prioritise daily work tasks. Desirable: support worker or mental health experience, preferably in ward or community settings. Other Requirements Employment requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of requiring a new check.
