Advanced Nurse Practioner (ANP) / Advanced Clinical PractionerCricklewoodGeneral Practice are looking for AdvancedNurse Practioner to join our team for 3 days per week, on Wednesdays, Thursdays andFridays.We offer:Excellent salaryand benefitsNHSPension5weeks Holiday and 1 week Study Leave (pro rata)Defined promotion and development pathwayFully computerised (EMIS Web) paper light practiceFull supportof an experienced clinical and administration teamFor enquiriesor to arrange an informalvisit, please email:Jennifer.mccalvey@nhs.netMain duties of the jobThe main duty of the role is to provide care for patients presenting withacute, non-acute and chronic medical conditions.This will involve taking the patient history, conducting physicalexaminations as appropriate, formulating differential diagnoses and ensuringrelevant follow up, treatment plans or next steps are taken.About usCricklewood GeneralPractice is locatedin newly refurbished premises close to Cricklewood Station.We currently have a patient populationof 5,000 and serve an ethnically diverse urban patient population.We have an established, friendly andsupportive multi-disciplinary team.We are supportive oftraining and development and our ideal candidate is someone who is looking tolearn and grow in their role.Job responsibilitiesJob responsibilitiesWorkwithin the professional Code of Conduct and other directives as defined by theNMC.Workautonomously and be accountable for own professional actions.Beresponsible for remaining up-to-date with CPD requirements to maintainprofessional registration with the NMC; including revalidation every threeyears.Ensureown educational commitment is at least sufficient to maintain CPD requirementsand to ensure own mandatory training certificates are current, in line withorganisational policy.Provideclinical leadership, support and training to colleagues and students.Demonstratea clear understanding of the accountability and legal implications of the ANProle.Participatein a regular performance review with the Senior GP, using an agreed competencyframework appropriate to the individuals scope of practice.ClinicalResponsibilitiesUndertakeclinical nursing practice at an advanced level using expert knowledge andclinical skills to deliver holistic care to registered patients.Assesspatients with a range of acute, non-acute, and chronic medical conditions.Take ahistory, conduct appropriate physical examinations, formulate differentialdiagnoses and carry through management treatment plans ensuring relevantfollow-up.Highlightany gaps in knowledge/training needs to the Senior GP and allow them theopportunity to offer support in order to fulfil the duties of this post.Remainup-to-date with clinical guidelines to ensure that practice remains evidencebased; particularly guidelines for the management of chronic diseases andnational/local guidance/ICB formularies when prescribing.Whereappropriate make referrals to other healthcare professionals in primary andsecondary care and arrange hospital admission for applicable cases.AttendMDT meetings and maintain effective relationships with multi-professionalcolleagues; such as the care co-ordinators, community palliative care team,district nurses, specialist nurses, mental health and social care.Undertaketelephone triage work and provide telephone advice calls/signposting to otherservices as appropriate.Undertakeconsultations in the most appropriate setting, using various consultationtypes; including, but not limited to, face to face, video, telephone,e-consultation.Undertakemedication reviews in line with local medicines management guidelines and withthe support of the locality pharmacist.Ensureclinical practice is safe and effective and remains within the boundaries ofcompetence.Advisepatients on general health care and minor ailments, with referral to othermembers of the primary and secondary health care team as necessaryWorkin line with organisational policies and procedural guidelines.Attendand participate in practice meetings as requiredTakepart in clinical audit to continually review and improve the quality of careprovision.Prescribetreatment in line with local and national guidelines and ICB formulary.Youwill be required to undertake all other duties that may be reasonably asked ofyou.Person SpecificationQualificationsRegistered Nurse with valid NMC registrationMSc or PG Diploma in Advanced Clinical PracticeEvidence of ongoing professional developmentExperience with EMIS Web clinical systemExperienceWorking within a Primary Care SettingInterest or expertise in specific long-term condition managementInterest or expertise in womens healthAdditional language skillsDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
