What you will do: Conduct comprehensive Care Act assessments, evaluate mental capacity, and engage in Mental Health Act duties. Develop and implement s117 discharge plans in collaboration with individuals and their families. Undertake regular reviews to ensure service user’s needs are met and adjust care plans as necessary. Address safeguarding concerns promptly and effectively, prioritising the wellbeing of service users. Provide duty cover as part of the team to manage immediate risks and crises. Foster relationships with internal teams, including those focusing on learning disabilities, physical disabilities, and services for older adults. Requirements of the Social Worker: A degree or equivalent qualification in social work. Registration with Social Work England. Significant frontline experience within UK Social Services as a qualified Social Worker is essential.
