Job Overview The post holder provides clinical leadership and support to the community nursing teams, managing a wide range of complex and sometimes conflicting priorities. They implement and ensure compliance with policies and procedures, maintain safe staffing across outpatient and community services, and deliver high‑value, family‑centered, responsive, and efficient patient care. Responsibilities Facilitate and provide the delivery of a high standard of patient care in conjunction with the child and their family/carers. Support and develop family‑centered care. Promote interagency multidisciplinary working by developing joint action plans, protocols, and pathways to ensure continuous quality improvement for service needs. Ensure that the needs of the patient are placed at the centre of care delivery. Lead and motivate the team, supporting prioritisation of caseloads and self‑management. Support all team members in teaching and assessment, and enable the wider nursing team to support families/carers to carry out specific nursing care for a variety of conditions. Work in collaboration with parents/carers, acting as an advocate for families, providing care to children and young people in accordance with the child’s plan of care to meet nursing and therapy needs, and liaise with all members of the multidisciplinary team when changes occur.
