We are an inspiring and dedicated team of Social Care practitioners focused on supporting adults with a Learning Disability to live independent and fulfilling lives. We would like to appoint either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G). You must be able to work well with a diverse team and embrace an integrated Health and Social Care approach to assessment and support in the community.
