We are an inspiring and dedicated team of Social Care practitioners focused on supporting adults with a Learning Disability to live independent and fulfilling lives. We are proud of our multi‑disciplinary working environment and our positive relationships with Health colleagues, achieving better outcomes for the adults we support. We would like to appoint either a Supported Assistant Practitioner (Scale F) or an experienced Assistant Practitioner (Scale G). You must be able to work well with a diverse team and embrace an integrated Health and Social Care approach to assessment and support in the community.
