PCN Advanced Clinical Practitioner

This is an exciting opportunityfor an Advanced Clinical Practitioner to join South Lincoln Healthcare PCN aspart of our expanding clinical team.The post holder will work withinand across our 7 member practices (Bassingham Surgery, Branston &Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, CliffVillages Medical Practice, The Heath Surgery, Washingborough Family Practice)along with undertaking work for the PCN.It is anticipated that the post holder will mainly work across 3-4practices, with a need to cover other practices on occasion, depending on need.The role will support the PCNspassion for providing an all-encompassing approach to personalised care andhealth inequalities across the PCN.Part time/job share would be considered.The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.It is anticipated that interviews for this role will take place on the 28/29 January 2026.Main duties of the jobThe post holder will be anintegral part of both the PCN and General Practice Teams, and will workalongside GPs, Practice Nurses, HCAs, GPAs, Clinical Pharmacists, PharmacyTechnicians, Occupational Therapists, Social Prescribing Link Workers, Health& Wellbeing Coaches, Care Coordinators, Mental Health Practitioners and MusculoskeletalPractitioners.The role will mainly consist of workingwithin our member practices to enhance and support day to day access andcapacity by providing a mixture of on the day and routine consultations whichmay be face to face, telephone, online or home visits. Time will be allocated for associated administration.In addition, the postholder will spend a smaller proportion of their time supporting the PCN by workingwith our care home team. This would involve supporting with triage, telephoneconsultations and visits to care homes.The role willrequire the ability to work flexibly to meet the needs of the organisation andthis may include working outside of core General Practice hours or within ourEnhanced Access service.About usSouth Lincoln Healthcare is a forward-thinking,innovative Federation of 7 GP practices, covering a population of just over 50kpatients in the South Lincoln Healthcare Primary Care Network area.By working together our ambitionis to strengthen and stabilise general practice.Our aim is to continue to providehigh quality healthcare to our patients in the community, whilst taking upopportunities to improve and extend our services.South Lincoln Healthcare issupportive of all staff and strives to cultivate inclusivity and a sense ofbelonging.PLEASE NOTE: We reserve the rightto close this vacancy early should we receive a sufficient number ofapplications.Job responsibilitiesIntegrate and work within our member practices to enhance and support day to day access and capacity.Provideadditional capacity support for the PCN care home team.Assess and triage patients of all ages (including children), including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways or make necessary referrals to other members of the Primary Care team.Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathologyand take appropriate action.Use complex decision making to inform thediagnosis, investigation, complete management of episodes of care within abroad scope of practice.Activelytake a personalised care approach to enable shared decision making with thepresenting person.Completethe relevant training in order to provide multi-professional, clinical practiceand CPD supervision to other roles within Primary Care, for example, firstcontact practitioners and personalised care roles.Undertakeclinical audit and quality improvement work across the PCN.Optionalfor the right candidateProvide programme leadership for our Cancerand/or Cardiovascular Disease ProgrammeProvide leadership for practice nursing teams(i.e. the importance of SMI health checks, elderly health checks, LD healthchecks)Provide support with the PCNs researchprogrammeBeresponsible for the day-to-day planning of personal workloads.Followorganisational policies, protocols, procedures, and standards.Report any incidents, accidents or complaints in a timely manner, taking appropriate action where necessaryDevelop themself and the role through participation in clinical supervision, training and service redesign activities.Contribute to a patient safety culture throughreporting and investigation of incidents and undertaking proactive measures toimprove patient safety.Maintainaccurate clinical records of all patient consultations and related work.Supportin the delivery of enhanced services and other service requirements on behalfof the PCN.Participatein the management of patient complaints when requested to do so and participatein the identification of any necessary learning brought about through clinicalincidents and near-miss events.Undertakeall mandatory training and induction programmes.Attenda formal appraisal with their manager at least every 12 months.
