Secondary Care Tobacco Treatment Advisor To work with the onsite tobacco treatment service and specialist to provide behavioural support and treatment in relation to tobacco smoking. To support the delivery of the service key performance and reporting indicators for tobacco dependency To support the day to day running of the onsite tobacco treatment service to ensure safe, well organised and evidence based care is offered routinely to all identified smokers and provided to all that access the service. Provide support in a range of settings as required for the delivery of smoking cessation service. Main duties of the job In this role, you will independently organise and prioritise your workload within a busy team, ensuring high standards and a commitment to equality, diversity, and inclusion. You will take referrals, including self-referrals, and work with patients to identify the most appropriate tobacco dependency treatment pathway while engaging individuals and groups from target communities. A key part of the role involves supporting clients to make informed health choices, encouraging sustainable behaviour change, and delivering evidence-based stop smoking interventions through one-to-one sessions, groups, drop-ins, telephone, or online support in line with the NCSCT Standard Treatment Programme. You will work collaboratively with professionals and partner agencies to embed smoking cessation into routine care, maximise health outcomes, and reduce inequalities, while providing high-quality, flexible, and cost-effective services across community and healthcare settings. The post also requires maintaining accurate, GDPR-compliant records, managing a client caseload via appropriate ICT systems, completing administrative tasks such as contact calls and appointment coordination, and supporting the development and improvement of services. You will help organise service workload, attend stakeholder meetings, complete relevant training, and contribute to the development of local referral pathways. Job responsibilities Duties and Responsibilities Organise ownworkload and work unsupervised in a busy team. Record and reportactivities and results. Develop andsustain tobacco dependency support and referrals into the Local Communityservices. Travel isrequired within the designated service location, including occasional regionaltravel. The tobaccodependency service will operate seven days each week. Thispost will be under the line management of the Project Lead supervised by theHead of the Smokefree Durham Service. The role will be varied, challenging, andrewarding as the services develop and change in response to the needs of ourclients and our Public Health Commissioners. Youwill be a motivated, passionate, organised, and proactive individual whocommunicates well with a variety of people. KeyResponsibilities: Motivate, empower and enable clients to makeinformed choices regarding their smoking addiction, and support in theirsetting of realistic sustainable goals that lead to their quitting smokingtobacco Deliver evidence-based stop smokinginterventions, these may include: one to one clinics, drop ins, group sessions,telephone, or on-line support, these should be in accordance with the NCSCTStandard Treatment Programme. Work with other professionals and agenciesto embed smoking cessation as part of routine client care. Maximise the impact of smoking cessation onhealth gains and reduce health inequalities. Provide high quality, efficient andcost-effective services throughout a diverse range of community and healthsettings. Provide a flexible, adaptable stop smokingservice that meets the needs of our clients including those who have mentalhealth conditions and/or problems with drugs or alcohol Ensure all client and service records areGDPR compliant, up to date, accurate and satisfies the ABL Health documentationpolicies. Successfully manage and maintain a caseloadof clients and use appropriate ICT systems to ensure continuity of care Provide effective administration along ourclient journeys in regard to client contact, appointment bookings,confirmations or cancellations. Identify opportunities to develop and improve services for your clients. Assist in the organisation of the servicesworkload, ensuring effective communication with your colleagues Attend stakeholder meetings as required. Attend all relevant training, internal andexternal to support the role. Skillsand Competencies Required: Understand smoking addiction and the impactit has on peoples health Trained (or willing to train) as an NCSCTcertified stop smoking practitioner Positive and proactive outlook with theability to motivate, engage and support others to achieve the highest standardsand outcomes.
