We are seeking to recruit a positive, caring and self-motivated individual to take on the role of Clinical Pharmacist.We are welcoming both full time and part time applications for this position. Please get in touch if you'd like an informal discussion prior to submitting an application.Main duties of the jobWorking as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients within your current competence, using your expert knowledge of medicines for specific disease areasBeing responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacyProviding specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients and to help in tackling inequalitiesSupport the leadership team on ensuring person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement is achieved, whilst contributing to the quality and outcomes framework and enhanced servicesSupport patients to take their medications to get the best from them, reduce waste and promote self-care where possibleSupport the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulationBeing part of a professional clinical network and have access to appropriate clinical supervision.About usWe level in Pharmacy (Or equivalent)Registered with the GPHCFaculty membership of the Royal Pharmaceutical SocietyIndependent prescribing or working towards/intent of gaining an independent prescribing qualification.ExperienceIn depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.Experience of multi-disciplinary workingClear understanding of professional responsibility and accountabilityKnowledge of current clinical evidence based practice.Experience of delivering patient care in a primary care, urgent or community setting, or willing to
