CNWL and West London Trust are hosting a joint recruitment drive for new posts within our Teams. Make a difference to those who require Urgent Community Response in their own homes – with enthusiasm, energy and passion – we are looking for Band 6 Registered Nurses, Paramedics or Therapists who have experience in community or tertiary services and want to continue developing as independent practitioners. (Newly Qualified staff cannot be considered for this role). Please note interviews will be face to face only. There will be 2 interview dates in January (6th and 20th January) to facilitate the recruitment of these posts. Main duties of the job Due to an opportunity to grow our teams we are aiming to recruit the best team players who will be passionate about providing care, proactive, engaged and committed to delivering an excellent standard of care in the home to those in need. Community experience is not essential but applicants will need to demonstrate the Trust core Values for CNWL and West London Trust and that they understand the issues involved in working in people’s own homes, working alone and being able to work outside of regular working hours, weekends, public holidays and face to face visits will be essential. If successful you will be providing holistic care to service users in their own homes and across our pathways of Urgent Community Response/Hospital at Home and Discharge to Assess. You will be required to be flexible, be team and patient focused and be able to work independently as needed. This position will require you to visit various locations throughout the London Boroughs of H&F, K&C and Westminster. We run a seven-day service and our working hours are 8 am until 10 pm/365 per year, which you will need to be able to fulfil required shift patterns including weekend working and out of hours working. Detailed Job Description And Main Responsibilities To assess, plan, implement and provide evaluation of health care needs in conjunction with patients, their carer’s and other appropriate members of the multi professional team. To have agreed areas of responsibility and work as part of the team. To provide nursing care to patient in the community. To see patients with potentially undifferentiated diagnosis including; objective assessment, physical assessment, treatment, follow‑up and developing care plans. To ensure all patients on the caseload are managed well and discharged safely either to their GP or for further ongoing assessment, rehabilitation treatment and care. To be responsible for keeping up to date with current issues and events in Primary and Secondary Health Care. Liaising with community services partners. To be an active member of the team and the team’s development. Person specification Education Essential criteria Registered Nurse/Paramedic/HCPC – current registration Evidence of ongoing and recent post-registration training and education. Desirable criteria Accredited post-registration qualification in long-term condition management and/or physical assessment skills – desirable Qualification in teaching or practice-based learning Experience Essential criteria Post-registration clinical experience in either acute (A&E, ICU) or UCR similar community setting Proven ability to prioritise, manage own patients and undertaking complex assessments autonomously Community nursing experience Experience of participating in Clinical supervision Evidence of innovation in practice Desirable criteria Experience of working collaboratively with a variety of agencies and professionals (desirable) Experience of undertaking complex assessments and providing care to people with long-term conditions Experience in managing, auditing and evaluating clinical nursing services Knowledge Essential criteria Knowledge across a range of patient conditions, including frailty, medical/surgical/cardiac – acute on chronic illness. Knowledge of current NHS organisational structures, risk assessment and personal accountability. Desirable criteria Knowledge of clinical governance processes. Skills / Aptitude Essential criteria Basic IT skills including use of Microsoft Office package and email Ability to communicate highly complex information both verbally and in writing Ability to adapt to changing needs of service Able to manage difficult situations Desirable criteria Advanced patient assessment skills or working on and developing these skills (desirable) #J-18808-Ljbffr
