30 hours per week. Hours and shift patterns to be discussed at interview. Role Summary The post is responsible for managing planned programmes of care for patients in domiciliary and clinic settings. A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in cooperation with other members of the health and social care teams, and voluntary and statutory agencies. Working for Our Organisation Our Community Nursing Team supports the needs of housebound patients living on the Wirral, providing valuable care that helps people manage long‑term conditions, maximise independence and prevent or safely discharge patients from hospital. The post will be based in the Wallasey locality, working with the Liscard Community Nursing Team. Key Responsibilities Work autonomously and as part of a team, exercising sound clinical judgment across the community care interface. Plan, implement and evaluate effective nursing care for patients in domiciliary and clinic settings. Co‑operate with multidisciplinary health and social care teams as well as voluntary and statutory agencies. Support flexible working arrangements and maintain flexibility to adapt to changing Trust needs. Application Requirements English proficiency sufficient to fulfil all spoken aspects of the role with confidence. References that can verify employment/education history over the last three years; referees must have a valid business email address. Compliance with Part 7 of the Immigration Act 2016 in terms of English language evidence. Student Visa Policy Full‑time permanent vacancies cannot be filled by individuals on a Student visa unless they have: applied for a Graduate visa, orcompleted the course and applied for a Graduate visa before the anticipated start date of employment. Compensation Salary is in line with national NHS Terms and Conditions and is pro‑rata for all part‑time positions. Relevant experience may influence salary offers.
