Locum Social Worker – Hospital Team (5 Years Hospital Experience Required)

Team: Hospital Discharge Team
Pay Rate: £32 - £34 per hour (dependent on experience)
Contract Type: Locum / Temporary

Are you an experienced and passionate Social Worker looking to make a real impact in a dynamic hospital setting? Our client in Central London is seeking a dedicated Locum Social Worker to join the Hospital Team. This is an exciting opportunity to support individuals and families at a critical point in their care journey and be part of a forward-thinking, supportive team
Key Responsibilities:
*
Undertake assessments under the Care Act 2014 to facilitate timely hospital discharges.
*
Work closely with health professionals, families, and community services to develop effective discharge plans.
*
Apply strengths-based, person-centred approaches to ensure positive outcomes for service users.
*
Ensure safeguarding concerns are identified and managed in line with legislation and local procedures.
*
Maintain accurate case records and documentation in line with council and professional standards.
Requirements:
*
Qualified Social Worker (Degree/DipSW/CQSW)
*
Current registration with Social Work England
*
Previous experience in hospital discharge or adult social care services
*
Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding protocols
*
Ability to work under pressure in a fast-paced, multi-disciplinary environment
