Overview 37 hours per week. These are exciting times on the Isle of Wight within Adult Social Care. The service has redesigned to enhance strength-based practice, working with the community and our third sector partners, bringing key teams together including Adult Safeguarding. We currently have two Social Worker vacancies in the Wellbeing & Access Hub, supporting people at first point of contact. This role works alongside Outreach and Occupational Therapy colleagues to reduce the need for long‑term care, by arranging support through reablement services to help the person regain independence. The role offers information, advice and signposting to services in the third sector. You will be part of the first point of contact for Adult Social Care, taking referrals by telephone, undertaking visits and Care Act Assessments. You will be responsible for timely assessments that are strength based, supporting planning that promotes and maintains independence, choice and autonomy for people. You will implement and promote departmental safeguarding procedures and adhere to departmental recording policy, as well as attend multi‑agency discharge planning meetings to ensure timely discharges. You will also be supporting carers and family members, sharing and advising where required, as well as offering Carers Assessments in line with the Care Act 2014.
