Job Overview We are looking for an enthusiastic, experienced Clinical Pharmacist to join our Clinical Pharmacy Team in the Barnstaple Alliance Primary Care Network. The role involves working in GP practices, managing patients with long‑term conditions, and supporting high‑quality, safe, cost‑effective prescribing. Hours: 25–37.5 hours per week, flexible. Location: Brannam Medical Centre, Barnstaple with travel to Fremington, Litchdon and Queens Medical Centres. Salary: £46,000–£59,000 WTE. Main Duties & Responsibilities Work as part of a multidisciplinary team, clinically assessing and treating patients using expert knowledge of medicines. Collaborate with clinicians, practice staff and the PCN to identify and manage a caseload of patients with long‑term conditions. Conduct clinical medication reviews, particularly for complex polypharmacy and patients with learning disabilities. Provide specialist expertise in medicine use, addressing public‑health and social‑care needs and tackling inequalities. Lead person‑centred medicines optimisation and quality improvement initiatives, including antimicrobial stewardship. Develop relationships with other pharmacy professionals across the PCN and wider health and social‑care system. Support integration of general practice with community and hospital pharmacy teams to improve patient outcomes. Participate in shared care protocols, clinical research and liaison with specialist pharmacists. Hold a professional clinical network and have access to appropriate clinical supervision. Eligibility & Requirements Experience Minimum of two years working as a pharmacist. Recent experience in a comparable role in General Practice (desirable). Experience of prescribing/clinical systems such as SystmOne, EMIS (desirable). Qualifications GPhC Registered Pharmacist. GPhC Independent Prescriber Qualification preferred; willingness to gain within 12 months considered. Essential Evidence of CPD. Ability to work flexibly, travel between sites, and undertake domiciliary visits. Clean valid driving licence. Excellent communication, IT, organisational and team skills. Strong analytical, decision‑making and teaching abilities. Ability to follow policy and procedure. Desirable Innovative thinking. People management experience. Analysis of prescribing data. Understanding of clinical governance and risk management. Knowledge of audit and research.
