What you will do: Conduct strengths-based assessments to understand the needs and goals of our customers. Collaborate with customers and their families to design and implement innovative reablement plans. Support the occupational therapy assistants and work alongside our supportive reablement team. Conduct client visits in various settings, including their homes or hospitals, to provide hands-on support. Maintain clear communication with all parties involved to ensure outstanding care delivery. To succeed in this role, you must hold a qualification in Occupational Therapy, have a minimum of two years post-qualification experience, and possess a valid full UK driver’s license, as travel to client’s locations is part of the role. Registration with HCPC is vital for this position.
